This does sound like a suicide attempt. I think a lot of people attribute being hogtied to the presence of an attacker when those who commit suicide are known to tie themselves so that they won't be able to back out.
Trivia: This makes an appearance on an episode of Succession, right down to the towel covering practice. That's how I found out about it to begin with.

I watched this immediately after a non-theatrical release became available. Was honestly disappointed. Not only for having no artistic value or merits as a film whatsoever (not like I expected it to be) but the fact that it wants to be edgy yet pulls their punches.
For example, in the "newborn porn" scene, you don't actually see anything. It just shows the lead's reaction to what was heavily implied to be fetus buggering. There's also a ton of cutaway shots from the violent scenes... like the penis-in-the-eye-socket, from what I remember he was just yanking on the guys head and we get a shot from behind of his dick exiting through the skull in cartoonish fashion.
I mean the target market of this movie are gorehounds looking for their next high, so why hold back? August Underground has a similar scene to the newborn porn one.
That being said, you're not missing anything if you don't see this. It's garbage. Some "controversial" films are actually "good" and worth experiencing like Salo, House That Jack Built, Martyrs, or In The Realm of the Senses. This one isn't.
They're a little harder to get into than most death metal bands. Their sound is harsher, more abrasive. And their look, well, is kinda off-putting.
As opposed to when I see Cannibal Corpse or Pig Destroyer, you can see yourself having a beer and casual chat with them and know underneath all those theatrics they're regular blokes. Heck, even Glen Benton/Deicide is known to be a chill guy despite all the anti-Christian stuff.
I think you're just focusing on the pooping cycle. The part where the parasite possesses the snail and creates this elaborate display with the eyes to draw its real intended host/target is what's fascinating.
