It's not all death. It's also mutilation, choking, blood, etc.

Mutilation is usually Amputee, Chocking and a little blood are within BDSM. Guro however means so much of it all that it either displays death directly or implicates death.

For clarification, you aren't wrong but may have missed the point. Nowadays guro is mostly gore, it was different (less extreme) at the start.
