China gets tested the most (even on a per athlete basis) yet has the least violations. Even with the privileges the US has for legalized doping under so-called "medical exemptions" that most other countries don't get, they still have more violations.
Why are China, Germany tested so much more?
The DDR was extreme innovative and good with their doping programs, and they were always ahead of the curve. That continued after the merge with West Germany, so German athletes receive a lot more scrutiny even though the major doping programs died on in the late 90s due to advances in testing technology.
Not sure. But I will note China has a very strong national policy against narcotics so unlike the US that loves that shit they actually want to stamp it out and may go above and beyond. That plus number of athletes. And also unlike the US they test all professional athletes, not just those who go to the Olympics while allowing paid professionals in various college or pro leagues to set their own rules (as the US does). Perhaps Germany also tests all professional athletes.

Unfortunately I have no correct answer either but I remember my father complaining about DDR weightlifters “eating male hormones”. This could be the underlying reason for Germany.
